To get appointed with long-term care insurers, simply
fill out the online appointment paperwork through our online NoMoreForms system.
You will be asked to use a client package code
to complete the online appointment and contracting process. Use one of
the following codes as noted:
- magabroker (Commissions
paid by carriers)
- magasolicitor (Commissions paid by MAGA)
Click on Get Appointed
below (a new window will launch),
complete the top section of the NoMoreForms
page
- first name
- last name
- Social Security Number (without
the dashes)
- password
- confirm password
- in the client package code field, type
appropriate code (see above)
Then click the "Logon To nomoreforms" button.
You will first complete the Appt Data Sheet Step 1,
then John Hancock - Broker, MetLife - Broker, etc., for the carriers you
wish to be appointed. Please print only the pages that require your
signature, sign and fax them to MAGA Limited with a cover sheet to the
"licensing department", fax number 503.627.9997.
When you have completed each carrier's appointment
paperwork, click on the rectangle at the very bottom of the last page of
each of the two documents to have them saved (it says either "Agree" or
"Save Your Info"). When they are completed, click on the "Submit Forms"
button. A page asks you to confirm that you want to submit the
appointment paperwork, click on the "Submit Forms" button one more time.
You will receive a confirmation number if this was done correctly. You
will not need to write the confirmation number down.
When you have completed the appointment paperwork,
please print only the pages that require a signature, sign and either
fax to 503.627.9997 or scan and email to
licensing@LTCexperts.com. The rest
of the documentation automatically gets submitted to MAGA Limited
online.
